The Rule of Three for Seizures – By Dr. Aman PS Sohal

Dr. Aman PS Sohal, one of the leading pediatric neurologists and a trusted seizure doctor in Dubai, the Rule of Three is a simple, parent-friendly method to understand when a seizure becomes an emergency. It helps families take timely action without panic.

1. Seizure Lasting More Than 3 Minutes

If your child experiences a seizure that continues beyond three minutes, immediately administer the prescribed rescue medication such as:

  • Buccal Midazolam

  • Intranasal Midazolam

  • Rectal Diazepam

And call an ambulance right away.

2. Three Seizures Within One Hour

If your child has three short seizures within a 1-hour period (example: 30 seconds, 10 seconds, etc.) without fully regaining consciousness in between, this is also an emergency.
Administer rescue medication and seek urgent medical care.

 3. Three Attempts to Recover but Still Abnormal

If after the seizure your child:

  • Doesn’t return to normal behaviour

  • Remains unusually sleepy

  • Has difficulty breathing

  • Or shows repeated abnormal jerks

after three minutes post-seizure, seek immediate medical attention.
